-payment 36 euros (cash or credit card only).

keep the blue form on you, until you recieve a yellow card, wich is valid 5 years.

to obtain the yellow card (taxi, minibus, ...), please address the following documents:

-blue form (avis médical)

-copy of your passeport

-copy of your (european) driving license card (both sides)

-yellow card

-SAE : Stamped enveloppe with your french address (company)
